Nonsurgical Treatment Options for Under-Eyes

Under-eye wrinkles, hollows, bags and puffiness can arise from a variety of factors. Aging, allergies, genetics, smoking, sun exposure, and even poor sleep can all contribute to a persistent tired appearance. Nonsurgical approaches work best for fine wrinkles, hollows, and small bags or mild puffiness caused by aging-related loss of collagen and skin elasticity. By targeting these changes directly, we can refresh the under-eye area without using invasive techniques.

Laser Skin Resurfacing

Laser skin resurfacing directly stimulates collagen remodeling while also improving skin texture. It works by delivering thermal energy to the skin, with columns of heat in a grid pattern. The laser energy causes tiny channels to form in the skin in order to create a wound healing response that causes immediate tissue contraction and over time builds collagen. This not only provides a degree of initial tightening but also encourages new collagen production, helping to firm aging under-eye skin. Additionally, through new collagen production, it helps reduce fine lines and crepey skin, further smoothing the under-eye area. Treatment intensity can be adjusted depending on the severity of your under eye wrinkles, thickness of your skin, and natural pigment of your skin.

Chemical Peel

A chemical peel works using similar principles to laser skin resurfacing, promoting turnover of damaged skin and boosting collagen production. The method however, involves applying a special solution to the thin eyelid skin with a small cotton tip. There is no heat or thermal energy applied. This solution absorbs into the skin, and over the course of a few days effectively removes damaged upper skin while building a robust collagen healing response. Dr. Schiedler can adjust the intensity of the peel, ranging from light surface peels to more intensive deeper treatments for more severe wrinkles.

MicroFat Grafting for Under-Eye Hollows (minimally invasive, no incisions)

As we age, the area beneath the eyes can lose volume. This creates hollowness, shadowing, and a tired look. Fat grafting uses a small amount of your own fat, gently collected from an area such as the abdomen or thighs through a tiny hole. The fat is purified and then carefully placed under the eyes to smooth the transition between the lower lid and cheek. Because the material comes from your own body, results look and feel natural and can last far longer than fillers. Autologous microfat grafting plumps up the skin while also improving skin quality over time. Fat grafting can be done alone or combined with lower eyelid surgery or laser resurfacing or chemical peel to further tighten the skin. Most patients return to normal activities within one to two weeks, and results continue to refine as swelling settles.

Benefits of Nonsurgical Under-Eye Treatment

  • No surgical incisions
  • Typically less downtime than lower eyelid surgery
  • Addresses multiple under-eye cosmetic concerns
  • Treatments can be combined, for synergistic results, such as microfat with chemical peel
  • Short individual treatment sessions
  • Customized approaches based on anatomy and skin condition
  • Natural-looking under-eye rejuvenation
  • Avoiding foreign substances in your body, such as filler, that can behave poorly over time

Who Is a Good Candidate for Nonsurgical Under-Eye Bag Treatment?

Nonsurgical under-eye bags treatments work best for patients with mild to moderate concerns caused by changes in the skin itself. For deeper concerns, such as, very large fat pad bulging, Dr. Schiedler may recommend a surgical approach (i.e., lower blepharoplasty). Generally, patients may qualify as nonsurgical candidates if they:
  • Have mild to moderate under-eye concerns
  • Prefer to avoid surgical methods
  • Have no medical conditions affecting the treatment area
  • Have realistic expectations about nonsurgical benefits
  • Can avoid sun exposure before and after healing

What to Expect During Your Under-Eye Treatment

  • Initial Consultation: Dr. Schiedler meets with each patient individually to assess the scope and cause of under-eye concerns. Based on the assessment, we can recommend either a nonsurgical or surgical approach.
  • The Procedure: We have you take an oral relaxing medication prior to arrival. We clean the treatment area and may apply a numbing medication or nerve blocks. The under-eye lower lids are treated once you are sleepy and comfortable. We apply a healing balm on your skin.
  • Recovery and Aftercare: You can return home right after your treatment. Most patients need a driver to take them home. Recovery times and experiences will depend on the type and intensity of your treatment modality. Your clinician will provide full details on what to expect and how to care for your lower lids.

Under-Eye Bags Treatment FAQs

How long is the downtime?

Downtime can range from 1 to 2 weeks. While the under-eye area heals, some swelling, redness, and mild discomfort (like a sunburn) are all normal. While each patient’s recovery pace will vary, common timelines include:

Chemical Peels:

  • Light Peel: 4-5 days
  • Medium Peel: 5–7 days
  • Deep Peels: 7-14 weeks

Laser :

  • Nonablative Laser: 3–7 days
  • Ablative Laser: 7 days
  • Deep Ablative Laser: 2–3 weeks

Microfat grafting for under eye hollows:

  • 1-3 weeks for mild swelling and bruising to resolve
Most patients will see dramatic improvements within the first week. Results continue to improve over the following months as collagen production continues, further improving skin texture, thickness, and quality. A single session will usually demonstrate noticeable improvements, but some patients may need a repeat treatment for additive benefits.
Natural aging will continue, but nonsurgical treatments still provide results that can last from months to years. Lighter treatments tend to provide benefits for 3–6 months, while improvements from deeper treatments can last longer (from 6 months to 3+ years, depending on intensity). Patient age, skin care, sun exposure, diet, and health will also all influence result durations. We typically recommend a maintenance treatment every 3-5 years to continue collagen production, and stay ahead of aging changes.
Costs will depend on the treatment type, intensity, and area, along with the number of areas treated. During the consultation, we provide a clear, no-commitment quote with clear price breakdowns.
Most procedures cause only mild discomfort, due to a combination of oral sedation and nerve blocks. With deeper under-eye treatments, there can be mild to moderate discomfort for 2-4 hours post treatment.
