Payment and insurance information
Payment is due at the time services are rendered. This means we will check your insurance coverage for eligibility and benefits prior to your visit. If you have health insurance, your contract with your insurance plan determines the amount of your copay, coinsurance, and deductible. We are required by law to collect this portion. Please understand that this amount will be an estimate until an explanation of benefits (EOB) is received from your insurance plan. We will do our best to collect the correct amount. You will be refunded or billed if there is any difference.
Self Pay Fees
We strive to make our services affordable to those who have no insurance, but unfortunately we are not able to set up payment plans. Payment is due in full at the time services are rendered.

No Show Fees
We understand that unexpected events can occur. We encourage you to communicate with us if you are unable to keep your appointment and we request that you kindly give us 24 hours’ notice. In an effort to keep our schedule open and flexible for all of our patient’s desired appointment times, a first no show or cancellation will be charged $50. A second no show or cancellation will be charged a $150 fee prior to rescheduling. A third no show or cancellation may result in dismissal from Oasis Eye Face & Skin.
There is a $30 fee for filling out forms such as DMV, FMLA, SDI, etc.
